CCHE celebrates 2025 accomplishments and collaborative partnerships

As 2025 comes to a close, we reflect on innovative projects that highlight our work across CCHE’s five focus areas. We look forward to continuing our efforts in the new year, using data and insights to strengthen programs and initiatives that aim to improve health and support the well-being of communities across the United States. Please read on for highlights of projects that wrapped up in 2025.

An online community health data tool

data platform map.jpgCCHE has been working on a Community Health data platform for Kaiser Permanente that uses publicly available data on health factors and health outcomes, emphasizing social and environmental factors affecting health. This year CCHE used the data platform to help complete the 2025 community health needs assessments for all communities served by Kaiser Permanente. Explore the platform here

 

Quality improvement support for small medical practices

iStock-1334660226.jpgEQuIP-LA was a two-year collaborative launched in 2023 to support 31 small, independent practices in Los Angeles that serve over 50,000 Medicaid patients. EQuIP-LA provided funding, technical assistance, individualized coaching, and data support through a train-the-trainer model. CCHE’s evaluation found that most practices improved performance on clinical measures and strengthened foundational primary care capabilities.  

A toolkit for evaluating health tech innovations

TechToolkit copy.jpgCCHE partnered with the California Health Care Foundation to create an evaluation toolkit for digital health companies, particularly for innovations serving patients enrolled in Medicaid. This resource can help companies evaluate new services and products when piloting programs with healthcare organizations. While the toolkit was built for digital health companies, much of its content is relevant for a broad range of organizations and contexts.

 

A comprehensive review of telehealth expansion

Picture1.pngSince 2020, CCHE has partnered with the California Health Care Foundation to evaluate telehealth initiatives aimed at ensuring accessible and effective care delivery in the safety net. CCHE’s latest telehealth report highlights the significant progress made, opportunities ahead, and tailored roadmaps for key partners. CCHE’s review found that California has made notable strides in telehealth policy, implementation, research, and consumer engagement, and that disparities persist for patients who use Medicaid, non-English speakers, and rural residents.

A 6-year partnership with the Cooperative Extension System

WCC.pngCCHE partnered with the Cooperative Extension System to evaluate its Well Connected Communities initiative. The initiative, funded by Robert Wood Johnson Foundation, leveraged Extension’s expansive reach to advance health equity and well-being across the United States. Over the course of this six-year initiative, CCHE produced several assessments and tools. In 2025, CCHE released its final evaluation report, detailing progress in systems change and advancing health equity and well-being at the national, state, and local levels.
